Published in 1988, this paper by the Adam Smith Institute (a Neoliberal think-tank), outlines the steps needed to transform the NHS from a socialised system into an insurance based system.

Around the same time  Oliver Letwin  authored  Britain's Biggest Enterprise , also setting out plans for the piecemeal privatisation of the NHS.
